What we collect
We collect the condition you select, your country of residence, name and email address, your consent, and any WhatsApp number or information about your situation that you choose to provide. Please share only what you want the partner clinic to review.
The form also records the enquiry date, page URL, submission identifier and timing information. A hashed version of your IP address is used for abuse prevention and rate limiting.
Why we collect it
We use these details to pass your enquiry to the partner clinic in Bangkok so its patient team can contact you and review your case. The legal basis is your consent, given by ticking the form's consent box.
Who receives it
Your enquiry is shared only with the partner clinic in Bangkok for this purpose. The clinic is in Thailand, so submitting the form means your information will be sent there. We do not sell your details.
Storage and retention
Enquiries are kept in private storage and delivered by email. Enquiries are retained for 12 months and then deleted. The same retention period applies to enquiry emails and copies held for this referral.
Your rights
You can request access to your details, ask for deletion or withdraw consent by contacting giebelhaus@mindfulconsulting.ch. Withdrawing consent does not affect processing that took place before you withdrew it.
Cookies and analytics
With your consent, this site uses Google Analytics 4, loaded through Google Tag Manager, to count visits and see which pages lead to enquiries. It sets Google Analytics cookies (names starting with _ga) and sends usage data such as pages viewed, device type and approximate location to Google. If you decline, no analytics cookies are set and Google receives only cookieless signals without identifiers (Google Consent Mode). The content of your enquiry is never sent to Google, only the fact that an enquiry was sent and the condition you selected. You can change your choice at any time with "Cookie settings" in the footer. Google may process data outside your country, see Google's privacy policy.
